Phlow Weight Loss Program – Terms of Service

By purchasing or participating in any Phlow weight loss service, program, or treatment plan, you agree to the following terms:

1. Medical Disclaimer

Phlow provides medical wellness services under licensed professionals. Weight loss treatments, including medications, IV therapy, and peptides, are not guaranteed to produce specific results. Outcomes vary based on individual health status, adherence, and lifestyle factors.

Participation requires full disclosure of medical history and may require lab work, consultation, and provider approval prior to treatment.

2. Payment Terms

All payments for weight loss services are due in full at the time of purchase unless otherwise specified in a payment plan agreement.

• Payments are non-transferable
• Pricing is subject to change at any time
• Packages must be used within the specified timeframe (if applicable)

3. Refund Policy

Due to the medical nature of services:

• All sales are final
• No refunds will be issued for services rendered, medications dispensed, or partially used programs
• If a client is deemed medically ineligible after purchase, a partial refund may be issued at Phlow’s discretion, minus consultation and administrative fees

4. Subscription / Membership (if applicable)

If enrolled in a recurring plan:

• Billing will occur automatically on a monthly basis
• A minimum commitment period may apply
• Cancellation requires at least 7 days’ notice prior to the next billing cycle
• No refunds will be issued for missed cancellation windows

5. Appointment & Cancellation Policy

• Appointments must be canceled or rescheduled at least 24 hours in advance
• Late cancellations or no-shows may result in a forfeited session or fee

6. Medication & Treatment Compliance

Clients agree to:

• Follow prescribed dosing and provider instructions
• Not share or misuse medications
• Report any side effects or concerns immediately

Phlow is not responsible for adverse outcomes resulting from misuse or non-compliance.

7. Results & Expectations

Weight loss is influenced by multiple factors including diet, activity level, metabolism, and consistency. Phlow does not guarantee specific results.

8. Privacy & Confidentiality

All personal and medical information is handled in accordance with HIPAA and privacy regulations.

9. Limitation of Liability

By participating, you agree to release Phlow, its providers, and affiliates from liability related to:

• Treatment outcomes
• Side effects or complications
• Failure to achieve desired results

10. Agreement

By completing your purchase, you acknowledge that you have read, understood, and agreed to these Terms of Service.
