Phlow IV Terms of Service
By purchasing or receiving IV therapy services from Phlow, you agree to the following terms:
1. Medical Services Disclaimer
Phlow provides medical-grade IV hydration and vitamin therapy administered by licensed healthcare professionals.
- IV therapy is intended to support hydration, recovery, energy, and overall wellness
- These services are not a substitute for primary care
- Results vary by individual and are not guaranteed
All clients must complete a medical intake and may be subject to provider approval prior to treatment.
2. Medical Eligibility
Treatment is provided only after evaluation by a licensed medical provider.
Phlow reserves the right to:
- Approve or deny treatment based on medical history
- Modify IV formulations or dosages for safety
- Refuse service if deemed medically inappropriate
3. Payment Terms
- Payment is due in full at the time of booking or service
- All services, packages, and add-ons are non-transferable
- Pricing is subject to change at any time
If purchasing a package:
- Sessions must be used within the stated timeframe (if applicable)
4. Refund Policy
Due to the nature of IV therapy services:
- All sales are final
- No refunds will be issued for:
- Completed IV treatments
- Partially used packages
- Missed appointments
If a client is deemed medically ineligible before treatment begins, a partial refund may be issued at Phlow’s discretion, minus any consultation or administrative fees.
5. Appointment & Cancellation Policy
- A minimum of 24 hours’ notice is required to cancel or reschedule
- Late cancellations or no-shows may result in:
- Loss of the session
- A cancellation fee
6. In-Home / Mobile Services (if applicable)
For concierge/mobile IV services:
- A safe, clean, and appropriate environment must be provided
- Phlow reserves the right to refuse or discontinue service if conditions are unsafe or unsuitable
7. Risks & Side Effects
You acknowledge that IV therapy may involve risks, including but not limited to:
- Bruising, discomfort, or irritation at the injection site
- Dizziness or lightheadedness
- Allergic reactions (rare)
By proceeding, you accept these risks and agree to notify your provider immediately of any adverse reactions.
8. Client Responsibility
You agree to:
- Provide accurate and complete medical information
- Inform staff of any medications, conditions, or allergies
- Follow post-treatment guidance
Phlow is not responsible for complications resulting from withheld or inaccurate information.
9. Results Disclaimer
IV therapy supports wellness but does not guarantee specific outcomes. Results depend on individual health, hydration status, and lifestyle factors.
10. Privacy & Confidentiality
All personal and medical information is handled in accordance with HIPAA and applicable privacy laws.
11. Limitation of Liability
By receiving services, you agree to release Phlow, its providers, and affiliates from liability related to:
- Treatment outcomes
- Side effects or complications
- Failure to achieve desired results
12. Acknowledgment & Agreement
By completing your purchase or receiving IV therapy, you confirm that:
- You understand the nature of IV therapy services
- You consent to treatment by licensed professionals
- You have read and agreed to these Terms of Service
